Setting up an account with Urodynamic-Supplies.com can be done online or offline.

To set up an account online, simply click the following link and then look near the bottom for the link titled “Create Account”.  It’s instantaneous so you can immediately place an order using a credit card. 

To set up an account offline, please download this application form (pdf format here) and fax it to 281-727-0250.  Once approved and you have received your login information by email, you can then place orders online or offline by using the fax order form(pdf format here) .

For Payment Terms, we will need additional information as specified on the Application but you can also opt for Net 30 Days once you have placed 4 orders and paid in advance.
